Protect people first
Stop safely, assess injuries and avoid standing in moving traffic. Follow dispatcher or responder instructions. Move vehicles only when it is safe and lawful to do so. Use hazard lights or other warnings when appropriate.
Exchange and document information
Local law determines required exchanges and reporting. Common information includes driver names, contact details, license and vehicle information, registration and insurance. Photograph the overall scene, vehicle positions, damage, road conditions and relevant signs only when safe.
Keep observations factual
Record the time, location, direction of travel and what you observed. Obtain witness contact information if offered. Avoid confrontation and do not make public accusations. Responders and insurers will evaluate the available evidence.
Reporting requirements vary
Police response and state collision-report requirements depend on injuries, damage, vehicle status and jurisdiction. Some minor incidents may use an online or self-reporting process. Confirm the rule with the responsible police department and state motor-vehicle agency.
After leaving the scene
Retain report numbers, photographs, repair documents, medical records and communications. Notify the insurer according to the policy. Seek qualified legal or medical advice when appropriate; MetroPD.org does not provide either.
